Please nominate a UUFoM member(s)  whom you feel has made and continues to make a significant contribution to UUFoM. The contributions may not be generally well-known by members and friends of the congregation. You may nominate more than one person. Please use a separate form for each nominee. Submissions are due by October 13, at 1:00 PM. Our Unsung Hero will be announced at our Annual Meeting on October 20.
